24、veyone has of lee plg le lontol sa ,proellg fe lntrl ad-s f_ prrvet on, e四、施工過程中實(shí)施的有關(guān)監(jiān)理工作制度本工程施工監(jiān)理過程中，我們實(shí)行了以下幾種主要的監(jiān)理工作制度：1、工程報(bào)驗(yàn)制：每個(gè)工序完成后，由施工單位自查自檢合格后，報(bào)監(jiān)理方進(jìn)行核查，核查合格后方可進(jìn)入下道工序施工，主體共計(jì)報(bào)驗(yàn)90次。2、施工測(cè)量放線報(bào)驗(yàn)制： 對(duì)模板、砌體的軸線、標(biāo)高、截面尺寸等進(jìn)行復(fù) 核。要求施工單位在技術(shù)復(fù)核合格后報(bào)監(jiān)理方進(jìn)行復(fù)核。3、工程材料報(bào)驗(yàn)制：對(duì)各種進(jìn)場(chǎng)的材料進(jìn)行查驗(yàn)，對(duì)要求做檢測(cè)、復(fù)試的材料，均由監(jiān)理方人員見證取樣送檢，合格后方可

25、在工程上使用。對(duì)用于本工程 上的鋼筋、水泥、碎加氣砌塊等主要建筑材料抽樣送檢情況如下：(1)、硅標(biāo)養(yǎng)抗壓試塊 共113組均合格，其中 C40: 8組、C35: 13組、C30: 50 組、C25: 42 組。(2)、硅同條件抗壓試塊共16組均合格，其中C40: 2組、C35: 3組、C30: 4組、C25: 7組、其中抗?jié)B試塊 C30P6: 10組。(3)、鋼筋原材料檢測(cè)報(bào)告3份，合格證11份，原材料送檢合格。(4)、鋼筋焊接：采用電渣壓力焊，檢驗(yàn)12組，檢驗(yàn)結(jié)果均符合設(shè)計(jì)及規(guī)范要求。(5)、墻體工程：外墻、內(nèi)墻采用 200, 100厚蒸壓加氣混凝土砌塊，強(qiáng)度等級(jí)AA5.0,其中蒸壓加氣混凝土

36、核算工程造價(jià)，對(duì)工程量簽證，均以業(yè)主代表、監(jiān)理、承包單位三方簽字的 原始憑證為依據(jù)。7、工程進(jìn)度控制：與總進(jìn)度計(jì)劃相比，本階段工程進(jìn)度總體上符合計(jì)劃進(jìn) 度。在計(jì)劃進(jìn)度實(shí)施過程中，施工單位安排了足夠的人力和物力，保證了計(jì)劃進(jìn) 度的有效實(shí)施。8、安全、文明施工、工程質(zhì)量方面：針對(duì)施工現(xiàn)場(chǎng)實(shí)際情況，按照安全、文明施工有關(guān)規(guī)范、規(guī)章制度，嚴(yán)格要求施工單位做好安全、文明施工，弁督促 施工單位對(duì)現(xiàn)場(chǎng)施工人員進(jìn)行安全、文明施工教育、加強(qiáng)技術(shù)交底，落實(shí)安全、 文明施工規(guī)章制度及技術(shù)交底到各工種班組；在此過程中我監(jiān)理部已發(fā)監(jiān)理工程師通知單23 份，工程聯(lián)系單30_份。五、工程質(zhì)量形成的主要事件2012年9月11

37、日市建委對(duì)本工程進(jìn)行了執(zhí)法實(shí)體抽芯，其中有 6組混凝土芯樣 強(qiáng)度檢測(cè)達(dá)不到設(shè)計(jì)強(qiáng)度，根據(jù)穗建質(zhì)監(jiān)字201132號(hào)文件要求對(duì)達(dá)不到設(shè)計(jì)強(qiáng)度的樓層進(jìn)行擴(kuò)檢復(fù)檢，（于2012年11月8日至2012年11月20日采取15個(gè)芯樣進(jìn)行擴(kuò) 檢復(fù)檢，并對(duì)所剩的樓層抽取一組的芯樣進(jìn)行檢測(cè)）；復(fù)檢同時(shí)于2012年11月12日 至2012年11月21日對(duì)南沙城商務(wù)辦公B棟、商業(yè)騎樓北段工程樓層進(jìn)行了結(jié)構(gòu)實(shí) 體檢測(cè)，其中在實(shí)體檢測(cè)中有 2組混凝土芯樣強(qiáng)度檢測(cè)達(dá)不到設(shè)計(jì)強(qiáng)度。根據(jù)穗建質(zhì) 監(jiān)字201132號(hào)文件要求對(duì)南沙城商務(wù)辦公 B棟、商業(yè)騎樓北段，實(shí)體檢測(cè)不合格 樓層的混凝土柱各抽取16個(gè)構(gòu)件進(jìn)行復(fù)檢（于2013年1

40、宜大于15個(gè)芯樣， 因此于2013年1月11至2013年1月17日對(duì)不合格的樓層增大構(gòu)件的檢測(cè)數(shù)量，即 在原來所測(cè)15個(gè)芯樣試件的基礎(chǔ)上再增加15個(gè)芯樣試件數(shù)量進(jìn)行檢測(cè)，以致增大30個(gè)芯樣構(gòu)件檢測(cè)數(shù)量后的結(jié)果得出不合格樓層的強(qiáng)度推定值。根據(jù)廣東省建材產(chǎn)品質(zhì) 量檢驗(yàn)中心結(jié)構(gòu)檢測(cè)報(bào)告(報(bào)告編號(hào)GD2013-36-00046)二層柱上限值與下限值之 差K = 5.3Mpa大于5.0 Mpa,無法給出該檢測(cè)批混凝土抗壓強(qiáng)度推定值，因此于 2013 年3月14日對(duì)二層柱采用全數(shù)檢測(cè)，同時(shí)對(duì)該強(qiáng)度等級(jí)相鄰的三層柱采用抽取16個(gè)芯樣進(jìn)行批量檢測(cè)。二層柱豎向構(gòu)件共68條，對(duì)所剩38條豎向構(gòu)件進(jìn)行采用抽取三個(gè)芯

41、樣，對(duì)原來所測(cè)30個(gè)豎向構(gòu)件一個(gè)芯樣試件的基礎(chǔ)上再加多兩個(gè)芯樣，即對(duì)二層 柱進(jìn)行批量全數(shù)檢測(cè)。根據(jù)最終的檢測(cè)結(jié)果經(jīng)設(shè)計(jì)單位核算，發(fā)現(xiàn)該部分混凝土柱構(gòu) 件強(qiáng)度偏低，不符合設(shè)計(jì)要求；根據(jù)文件規(guī)定要求，設(shè)計(jì)單位的核算結(jié)論需經(jīng)審圖中 心審核。因此經(jīng)設(shè)計(jì)單位及審圖中心一致得出的結(jié)論：對(duì)不符合要求的21根墻柱進(jìn)行加固處理以滿足結(jié)構(gòu)的正常耐久使用，加固工程完成后，按照分部工程進(jìn)行專項(xiàng)驗(yàn)收。2013年7月17日經(jīng)施工單位、建設(shè)單位、監(jiān)理單位、設(shè)計(jì)單位等相關(guān)單位對(duì)此進(jìn)行了 該加固分部工程質(zhì)量的驗(yàn)收，驗(yàn)收結(jié)果為：該加固分部工程質(zhì)量均符合設(shè)計(jì)及規(guī)范要 求。六、在施工過程中存在的主要質(zhì)量問題及整改情況在本工程的整個(gè)施
